Contractor Pre-qualification Information - Renewal ApplicationsContractors are required to renew their prequalification application every three years. If you are not currently on the list of pre-qualified contractors, please visit the section on new applications at www.da.ks.gov/fp/contractor/newapps.htm. For companies currently on the list of pre-qualified contractors, we attempt to notify 2 to 3 months prior to the expiration date that it is time for a renewal the application. Whether you receive a letter or know your expiration date, please access the renewal form below and forward to DFM as directed along with the required attachments. This form replaces the AIA A305 and the AGC Document 221 that is required of new applications. Companies that do not get their renewal application in prior to their expiration date will be removed from the list of prequalified contractors and have six (6) months in which the renewal application will be processed. If the company waits longer than six (6) months to renew their prequalification, they must start over following the instructions for New Applications. Renewal form [.doc]
