16-Cycle-EE – If checked, indicates that the employee is employed for a period of 9 or 10 months.

A Eff Date: - Date on which the dependent/beneficiary’s Part A Medicate coverage took effect as listed in Personal Data in the Administer Workforce component.

Account Information – Summary information for this Flexible Spending account.

Address 1-3: - Dependent/beneficiary’s address.

Annual Pledge: - Annual amount of FSA contribution.

B Eff Date: - Date on which the dependent/beneficiary’s Part B Medicate coverage took effect as listed in Personal Data in the Administer Workforce component.

Before Tax Investment box – Savings plans are predefined as either a flat amount or percentage of gross. All savings plans for the State are before-tax.

Before Tax Investment box and After Tax Investment box – Indicates if the deduction is subject to income tax.

Beneficiary Percentage – This field is not used for KPERS Buybacks.

Benefit Plan: - Identifies the specific benefit program in which the employee is enrolled. For retirement, it could be P2 for new hires or P for those already in KPERS.

Benefit Program: - Benefit Program code corresponds to the employee salary range as of the deduction begin date. Defaults from Benefit Program Participation page. (KPERS RETIREMENT ELECTIONS, HEALTH BENEFITS ELECTIONS, GROUP TERM LIFE, FLEXIBLE SPENDING, OPTIONAL GROUP LIFE)

Benefit Program: - Benefit Program code corresponds to the employee salary range as of the deduction begin date. Defaults from Job Data in Administer Workforce.(DEFERRED COMPENSATION, KPERS BUYBACK)

Benefit Program: - Indicates the Benefit Program in which the employee is enrolled. (BENEFIT PROGRAM PARTICIPATION)

Birthdate: - The dependent/beneficiary’s date of birth. Date must be in MM/DD/YYYY format.

Box – The information you see depends on the benefit plans in which the employee is enrolled.

Change Reason: - Code identifying the reason an employee began or changed coverage.

City: - City in which the dependent/beneficiary resides.

Country – Country in which the dependent/beneficiary resides.

County: - County in which the dependent/beneficiary resides.

Court Ordered – Dependent information defaults from the Dependent/Beneficiary page.

Coverage Begin Date: - Date on which the coverage begins or ends.

Coverage Code: - Code that defines the level of coverage, such as employee only or family.

Coverage, Coverage Election or Participation Election: - Displays three coverage election options (Elect, Waive, or Terminate). Defaults to Elect.

Coverage Options: - Indicates the amount of Optional Group Life Insurance as a flat amount.

Date of Death: - Date of dependent/beneficiary’s death, if applicable. Date must be in MM/DD/YYYY format.

Deduction Begin Date: - Date on which the deduction begins or ends.

D, DepBenef Type, or Depend/Benef: - Base benefits administration types are Dependent, Beneficiary, or Dep/Benef (which is both).

Dependent/Beneficiary ID: - The system assigns a unique ID number to each dependent or beneficiary in sequential order.

Dependent/Beneficiary or Name: - A dependent/beneficiary’s name.The field is case sensitive.

Detail 0 – Defaults from Job Data.

Detail 1 – Defaults from Job Data.

Effective Date: - Benefits Program effective date (BENEFITS PROGRAM PARTICIPATION)

Effective Date: - Date on which the deduction begins or ends.

Election Date: - System defaults to date of data entry.

Empl Contribution Override: - Biweekly amount of FSA contribution.

Employee Status: - Displays whether the employee is active or inactive as of deduction begin date. Defaults from Job Data.

Event Date: - Date of the event causing the change in coverage.

Gender: - Identifies the dependent/beneficiary’s gender. If gender is not known, use either gender.

Health Provider ID – Unique number used to identify the employee or dependent’s primary care physician at time of enrollment, when applicable.

ID: - The system assigns a unique ID number to each dependent or beneficiary in sequential order.

Marital Status: - Identifies marital status of the dependent/beneficiary. If not known, use “Single.”

Medicare Entitled Date: - The date upon which the dependent/beneficiary will become eligible for Medicare coverage.

Medicare Number: - Dependent/beneficiary’s Medicare number as listed in the Personal Data in the Administer Workforce component.

National ID – Social Security Number of the dependent/beneficiary. System automatically fills in the dashes.

Occupation: - Identifies the type of job the dependent/beneficiary holds.

Payroll Status: - Indicates whether the employee is active or inactive as of the deduction begin date. Defaults from Job Data in Administer Workforce.

Phone: - Optional field for telephone number for the dependent/beneficiary. System automatically formats the numbers.

Plan Type: - Identifies the type of plan.

Postal: - Dependent/beneficiary’s zip code.

Relationship or Relationship to Employee– Identifies the dependent/beneficiary’s relationship.

Rollover Box - Data on this inquiry page defaults from job, personal, and employment data in Administer Workforce.

Same Address as Employee: Address – Click the check box on if the employee’s address is the same as for the dependent or beneficiary.

Same Phone as Employee: Click the check box on if the employee’s phone number are the same as for the dependent or beneficiary.

State: - State in which the dependent/beneficiary resides.

Total Percentage: - Ensures that the total percentage for all beneficiaries equals 100.
