Kansas Learning Quest Savings Program Payroll Deduction Form (.pdf) - Use this form if you are signed up for the Learning Quest program and need to change your payroll deduction. The form is in fillable .pdf format and once completed should be printed from your printer, signed and returned to your agency personnel/payroll person.
